An exciting opportunity exists for two enthusiastic and experienced practitioners to work for SalvoCare Eastern and be part of the Homelessness and Support Programs.
The Salvation Army SalvoCare Eastern provides a range of Housing & Homeless, AOD, Youth, Out of Home Care and Chaplaincy services for statutory and non-statutory clients with complex needs throughout Eastern Metro, Peninsula and Gippsland areas.
As an integral part of the team you will provide intensive case management to clients, working with the wider Salvation Army programs for integrated support, and enable linkages for clients into community. These positions are only based in the Eastern Metro Programs.
The Case Manager – Homeless Outreach Program will provide an assertive outreach model to people who are homeless or at risk of homelessness with a focus of assisting people with a disability to be linked into appropriate resources including the NDIS
